Intake form, in Turkish
The form below is in Turkish, and so is the work.
Therapy happens in language, and I work only in the language where I can hear the smallest hesitation. For me that language is Turkish, so the counseling, the first conversation, and the form below are all in Turkish. If Turkish is a language you are at home in, please go ahead. If it is not, I would rather you knew now than after 56 questions, and I would encourage you to look for someone who works in your own language. That is not a smaller kind of help. It is the right kind. What you share here is kept confidential within professional ethics and the law, and used only for the counseling process. That confidentiality has two limits — an imminent and serious risk of harm to yourself or someone else, and a lawful request from a competent authority — and both are set out in section 7 of the Privacy Notice.
If you are in immediate danger or having thoughts of harming yourself, please do not wait. Contact the emergency services in your country or go to the nearest emergency department. The intake is not a substitute for emergency care.
